New Students

The following information is needed to Register a new student:

1. Parent/Guardian ID

2. Proof of Arizona Residency Document. See registration packet.

3. Proof of Custody is needed for a student that does not live with both natural parents

4. Birth Certificate

5. Immunization Records

6. Exceptional Student Services, Special Services, and Accommodations

7. Withdrawal Form and Withdrawal Grades

8. Unofficial Transcript

9. AzMERIT/AIMS Test Results

10. Discipline Records
